Terms and Conditions

To cancel or reschedule time of services, please contact provider at 904.577.2440 or email at [email protected] to request change of services 3 days prior to scheduled appointment. If request is not rescheduled or canceled within the appropriate time frame, a surcharge for services other than Qualified Home Care Services will be billed half of the agreed final cost. A surcharge of scheduled missed appointments for Qualified Home Care Services, will be billed a minimum fee of 3 hours. Please contact us upon any concerns to avoid negligence of homecare.
